New Patient Info

Senior Health Associates is currently accepting new patients in all offices.  If you were previously on our waiting list and have not been contacted to set up services, please contact our office at:

We accept Medicare, Tricare 4 Life and most other insurance plans.  You can e-mail us to verify that we accept your insurance plan. We require that all patients complete our New Patient Forms prior to scheduling a appointment with our physicians.  

Once we have received your completed forms, we will verify your insurance and contact you to set up an initial visit.
