Medication Refills             See below for fee information

Prescription refills should primarily be requested during scheduled visits with your providers.  Please check your medication labels to see how many refills remain and the prescription expiration date prior to your appointment.  Expiration dates are usually found at the bottom of the prescription label.  You will see something like this "5 Refills Remain Prior To 05/15/2011".  This means that your prescription expires 05/15/2011 and you will lose remaining refills after that date.

If necessary, you may call our office and leave a message on the refill line (option 2) at any time.  We will phone your prescription in to your pharmacy usually within 2 business days if you have provided us with complete information.

You must include the following information with your request:                                      

If your prescription requires mailing (such as Duragesic, Fentanyl, Morphine, Percecet, etc) you will need to give at least 5 days notice or you may not receive your prescription before you run out of medicine.

Urgent refill requests made to our office staff or by paging the provider on call will incur a $25.00 fee.  Please utilize our medication refill line (option 2) to avoid this charge.
